You may download the Official Application for Ministeral Credentials below or contact the Network Credential's office at bethb@northwestministry.com for an application. Please sign and mail the application to the Network office along with the notarized release form(s), Code of Ethics form, Background Check Disclosure form and any necessary attachments.
The applicant is requested to submit with his/her application the following:
Completed and signed application with any necessary attachment or explanations to questions;
Transcript(s) of education completed;
Notarized Authorization form(s);
Signed Code of Ethics form;
Background Check Disclosure;
Two photos; and
A filing fee of $150 made payable to the Northwest Minstry Network of the Assemblies of God. (Please contact the Network office if this creates a hardship.)
The current deadlines for application are June 1, 2010 and October 1, 2010.
The application requires references that include your pastor, ministers acquainted with your ministry background, friends, and employers. All references should know the applicant well enough to answer a comprehensive questionnaire. NOTE: Full addresses for references are required on the application.
All applicants are required to take a written examination for the level of license that is applied for. After you submit your application, your exam will be scheduled by the Network office. Applicants are asked to use only their Bible during a portion of the exam; and may use the Northwest District Constitution and Bylaws for a part of the exam. A study guide of the required examination will be sent to you to assist you in preparing for the examination.
The summer examination date will be in August of 2010 and the fall examination will be in November 2010.
Successful completion of the examination or educational requirements does not automatically insure acceptance for a credential due to other factors that may be involved in the application process.

Please read the NWMN Ministerial Ethics Manual. This manual is for your reference. The attached Code of Ethics should be signed and returned to the Network office with your credential application.
